    Home ACCIDENT & INCIDENT REPORTING MODULE

    Definitions:

    ACCIDENT:- Employee or customer suffers a personal injury

    INCIDENT:- Near Miss & Undesired Circumstances

    Near Miss – Potential to cause injury or ill-health

    Undesired Circumstances – A set of conditions or circumstances with potential to cause injury or ill-health

    Training & Competency

    All staff must receive a health & Safety Induction

    All staff treating injured persons must be first aid qualified

    What must be reported?

    Work related accidents, which cause death to employees & members of the public

    Work related accidents which cause serious injury

    Diagnosed cases of certain industrial diseases

    Certain dangerous occurances

    Gas Incidents

    Reporting - Employees
    Arising out of or in connection with the work activity:

    An employee is killed

    An employee suffers a specific injury

    An employee suffers an over seven day injury

    An employee remains in hospital for more than 24 hours

    Accident Reporting - Public
    Arising out of or in connection with the work activity:

    A member of the public is taken to hospital directly

    A member of the public is killed

    Timescales for Reporting:

    Work related death or reportable injuries - notify the enforcing authority without delay

    Online or telephone HSE on 0845 300 9923

    A written report must be sent within 10 days

    Over 7 day injuries must be notified within 15 days

    Data Protection:

    Copies of the accident report forms must not be passed to any third party

    Disclosure of the accident report forms can only be made through the insurers
